Position: Medical Scribe
Location: PECHS, Khalid Bin Waleed Road, Karachi
Employment Type: Full-Time | In-Office
Shift: Night Shift / U.S. Healthcare Hours
Experience: 1–3+ years preferred

About the Position
We are looking for a skilled and detail-oriented Medical Scribe to join our healthcare documentation team. The successful candidate will work directly with our operations and scribe team to prepare accurate, timely, and clinically appropriate documentation for U.S.-based healthcare providers.
This position is ideal for candidates with experience in medical scribing, medical transcription, clinical documentation, or U.S. healthcare workflows.

Key Responsibilities
Prepare accurate SOAP notes, follow-up notes, physical examinations, wellness visits, and telemedicine notes.
Convert provider-patient encounters into clear, structured clinical documentation.
Accurately document HPI, ROS, physical examination, assessment, and treatment plans.
Review documentation for completeness, accuracy, grammar, and medical terminology.
Assign and verify appropriate ICD-10-CM diagnoses based on provider documentation.
Follow provider-specific templates, preferences, and documentation requirements.
Ensure documentation supports the diagnoses and services addressed during the encounter.
Identify missing or unclear clinical information and escalate appropriately.
Complete assigned notes within required turnaround times.
Work closely with the Scribe Manager and other team members to maintain workflow and minimize backlog.
Maintain strict HIPAA and patient confidentiality standards.
Support documentation related to preventive care, AWVs, quality measures, and care gaps when required.

Required Qualifications
1–3+ years of experience in medical scribing, medical transcription, or clinical documentation preferred.
Strong knowledge of medical terminology.
Excellent understanding of SOAP and outpatient clinical documentation.
Working knowledge of ICD-10-CM coding.
Excellent English grammar, spelling, and written communication.
Strong listening and comprehension skills.
Excellent attention to detail.
Ability to accurately interpret provider-patient conversations.
Experience working with an EHR/EMR system.
Ability to work under pressure and meet productivity and TAT requirements.
Strong computer skills.
Ability to work night shifts according to U.S. healthcare hours.
Willingness to work full-time from our PECHS office.

Preferred Skills
Candidates with the following experience will be given preference:
Experience working with U.S.-based physicians or medical practices.
Experience with Practice Fusion, Cerner, or similar EHR systems.
Medical transcription background.
Knowledge of Medicare Annual Wellness Visits (AWV).
Understanding of HEDIS and healthcare quality measures.
Familiarity with MEAT documentation principles.
Basic knowledge of medical billing and coding.
Experience handling multiple providers and high-volume documentation.
Previous experience in a medical billing, transcription, or healthcare BPO environment.
