Job Summary:

Concept Health Technologies Pakistan are seeking a proactive and empathetic Care Coordinator to join our growing team. This role is central to the patient journey, acting as the first point of contact for families and ensuring a seamless experience from initial inquiry to post-assessment care. You'll work closely with our clinical team, supporting scheduling, documentation, and communication, and guiding families through the clinical assessment and treatment process.

Key Responsibilities:

  • Serve as the first point of contact for patients and families, offering clear, compassionate guidance on the ADHD/Autism or other similar assessment process.
  • Manage scheduling and coordination of appointments with clinicians.
  • Respond to calls, texts, and email inquiries in a timely and professional manner.
  • Collect, organise, and follow up on required documentation, such as school or workplace reports and patient forms.
  • Onboard new patients, ensuring they understand the pathway and what to expect.
  • Provide clear information regarding assessment fees, available payment plans, and ongoing subscription services.
  • Support patients’ post-assessment, helping them understand their diagnosis, treatment options, and next steps.
  • Coordinate the medication pathway, including prescription reviews and follow-up care.
  • Act as a liaison between patients and the clinical team, ensuring smooth and timely communication.

Requirements:

  • Excellent written and verbal communication skills in English.
  • Strong organisational skills with attention to detail.
  • Customer-focused mindset with politeness, empathy and professionalism.
  • Ability to manage multiple tasks and priorities in a remote working environment.
  • Tech-savvy and comfortable using digital tools for communication and scheduling.

Working Hours: 12pm – 10pm
